Service

An AERS Nurse or Social Worker can visit a vulnerable person in their home to complete an assessment of their health and overall situation to determine that person’s eligibility for certain programs and services.As part of an AERS assessment, a resource list is shared with the client and referrals might be made for local case management, Medical Adult Day Care (MADC), or other services and supports as eligible. (Please note that some of these programs may have a “wait list.”) AERS representatives can re-assess the client each year or when there is a significant change in the client’s situation which might affect the type of services they need (client or family can call to request another evaluation as needed).AERS also conducts the assessment that helps to determine an individual’s eligibility for Community First Choice (CFC). CFC is a program that provides personal care support for individuals based on their needs and eligibility (CFC is only available to those who receive straight Medical Assistance – not SLMB or QMB) – AERS completes the initial assessment for CFC and it is then determined if the person will receive a “level of care” for this type of assistance. CFC assists chronically ill, frail elderly, or disabled people to remain in their own home by providing personal care assistance for activities of daily living such as bathing, dressing, eating and so on. For those who are approved for the program, personal care services are provided by an approved Agency Provider who is supervised by a nurse monitor. Personal Care Providers are not hired or employed through the AERS office.Referrals for AERS assessments may come from any source (self, family, physicians, MADC, VA, etc.). The program is voluntary (when a referral is made, the person has the right to accept the offer for an evaluation or to decline). Assessments are provided at no cost to the client or family.Contact the AERS office at 410-479-8055 to determine if an AERS assessment could benefit you or someone you know. When making a referral, please try to have as much information as possible so that we can determine if the person is eligible for services (name, address, phone number, contact info for the person’s Power of Attorney or other involved support person, date of birth and social security number if those are known, and the reason for your referral). We will then contact the client to tell them about AERS and offer an evaluation in order to tell them about programs and services that might make sense for them in their individual situation.
