Choptank Community Health Systems, Inc. is a private, non-profit community health center providing a full range of primary medical, dental, and behavioral health services in Caroline, Dorchester and Talbot Counties. CCHS is a participating provider with most major insurance companies and HMO’s and provides a reduced fee schedule for people who are uninsured or underinsured, based on financial need.

CCHS offers comprehensive family practice internal medicine, pediatrics, family dental care, women’s health, and behavioral health services through its 5 Medical and 4 Dental Centers on the Mid-Shore. The Migrant Health Project provides outreach services to meet the special needs of the migrant population. School-Based health services are provided in 9 Caroline County schools and 4 Talbot County schools. Oral health services are also available through School-Based programs in all three Counties.
